• Current Arizona LMT License. You must hold a valid Arizona Licensed Massage Therapist credential through the Arizona State Board of Massage Therapy. No exceptions.
• Individual Professional Liability Insurance. You carry your own individual malpractice coverage, separate from the company-level policy Desert Holistic Wellness LLC maintains. Minimum $1 million per occurrence. If you do not currently have this, it is straightforward to obtain and the annual cost is modest relative to what it protects.
• Professional Equipment. You arrive with a professional massage chair in good condition. We do not supply equipment.
• Professional Appearance and Conduct. You represent Desert Holistic Wellness LLC in someone else's workplace. The standard is high and consistent.
• Signed Contractor Agreement and W-9. Standard paperwork before your first assignment.
